My wife was terrified when I first brought up TRT.

I had done the labs. I knew my numbers were in the gutter. I knew I needed help. But when I told her I was thinking about starting Testosterone Replacement Therapy, she didn't hear "medical treatment."

The media has done a great job of terrified everyone about testosterone. We associate it with aggression, violence, and toxic masculinity.

But as a doctor, I need to separate the locker room bro-science from the actual medical reality. Because this myth is preventing thousands of men from saving their health, their energy, and yes—even their marriages.

The Origin of the Myth

"Roid Rage" is a real phenomenon. I won't deny that...BUT context is everything.

Roid Rage occurs in bodybuilders and powerlifters who are abusing illegal anabolic steroids at supraphysiological doses.

Let’s talk numbers. A healthy male produces about 6 to 7 milligrams of testosterone per week naturally.A guy on a specialized TRT protocol is usually taking 100 to 140 milligrams a week to replicate that natural level.

A bodybuilder on a "cycle"? They might be taking 1,000, 2,000, or even 3,000 milligrams a week, often stacked with other synthetic compounds that were never meant for human consumption.

When you blast the human brain with 50 times the natural limit of androgens, yes, you destabilize your neurochemistry. You become aggressive. You become manic.

But comparing TRT to steroid abuse is like comparing a glass of wine at dinner to downing three bottles of vodka in an hour. One is a nice accompaniment to life; the other is a disaster.

The Paradox: Who is Actually Angry?

Here is the irony that I see in my clinic every week.

The angriest, most irritable, most "snappy" men aren't the ones with high testosterone. They are the ones with Low Testosterone.

Think about the "Grumpy Old Man" stereotype. Why are they grumpy? It’s not just because the world is changing. It’s physiology.

When your testosterone crashes, a few things happen:

  1. Cortisol Spikes: Testosterone and Cortisol have a see-saw relationship. When T goes down, stress hormones often go up. You live in a state of low-grade "fight or flight."
  2. Sleep Suffers: Low T ruins sleep architecture. You wake up tired. And we all know how patient we are when we’re exhausted.
  3. Neurotransmitters Fade: Testosterone regulates dopamine (motivation) and serotonin (mood). Without it, you feel anxious and depressed.

When I treat a guy with low T, he’s usually on a short fuse. He snaps at his kids for being too loud. He gets road rage. He’s defensive with his wife. He’s running on fumes.

The "Estrogen" Factor

Here is another piece of science the gym bros miss.

Testosterone converts into Estrogen (Estradiol) in the body. You actually need estrogen for brain health, libido, and joint protection.

Guys who abuse steroids often take other drugs to completely crush their estrogen. Or, they take so much testosterone that their estrogen spikes to the moon. That hormonal rollercoaster is what causes the mood swings.
