Consultation Required: All treatments require an in-person consultation and evaluation by Dr. Abdullah or another qualified provider at our Southlake, TX clinic. Information on this Site about treatments doesn't guarantee you're a candidate for any specific therapy.

Payment and Cancellation: Payment terms, cancellation policies, and refund policies will be provided during your consultation and are subject to separate agreements. These policies may vary by service.

5. Privacy and Protected Health Information

Your use of this Site is also governed by our Privacy Policy, which explains how we collect, use, and protect your information. By using this Site, you consent to our Privacy Policy.

13. Governing Law and Dispute Resolution

These Terms shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions.

Informal Resolution: Before filing any formal legal action, you agree to first contact us at info@magnoliafunctionalwellness.com to attempt to resolve any dispute informally.

Arbitration: Any dispute that can't be resolved informally shall be resolved through binding arbitration in accordance with the American Arbitration Association's rules, conducted in Tarrant County, Texas. You agree to waive your right to a jury trial and to participate in a class action lawsuit.

Exceptions: Either party may seek injunctive relief in court to prevent actual or threatened infringement, misappropriation, or violation of intellectual property rights.

14. Severability

If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that these Terms will otherwise remain in full force and effect.

15. Entire Agreement

These Terms, together with our Privacy Policy and any other legal notices published on the Site, constitute the entire agreement between you and Magnolia Functional Wellness regarding your use of the Site.

17. Specific Texas Medical Practice Disclosures

Texas Medical Board: Dr. Farhan Abdullah is licensed to practice medicine in the State of Texas. Questions or complaints about medical care can be directed to the Texas Medical Board at (512) 305-7030 or www.tmb.state.tx.us.

Notice of Privacy Practices: A copy of our HIPAA Notice of Privacy Practices is available at our clinic and will be provided to you at your first visit.

Surprise Billing Protections: You have rights under federal and Texas law regarding surprise medical bills. Information about these protections is available at our clinic and upon request.
