Lab Testing 101: Every Woman's Hormone Panel Explained
Most women who come in with hormone labs already in hand are carrying results that can't answer the question they were meant to answer. Dr. Farhan Abdullah walks through what actually belongs on a woman's hormone panel, why cycle timing changes everything, and why a TSH alone isn't a thyroid evaluation. He also covers the metabolic and nutrient labs that routinely get skipped, and how to read your own results without falling into either the reference-range trap or the chase-the-number trap.

A woman came to see me last spring carrying a manila folder about an inch thick. Inside were three years of lab results from four different offices. She was 46, exhausted by two in the afternoon, waking at 3 a.m. for no reason she could name, and gaining weight around her middle despite eating the way she always had. Every panel in that folder had been read back to her the same way: everything looks normal.
I flipped through it while she talked. She was right that nothing was flagged. She was also right that something was wrong. Both things were true, and the reason they were both true is that almost nobody had ordered the right tests, and the few correct tests that had been ordered were drawn on the wrong day of her cycle.
This happens constantly. At Magnolia Functional Wellness, I'd estimate that half the women who come in for a hormone consultation arrive with labs already in hand, and most of those labs can't answer the question they were meant to answer. So let's go through it properly. What belongs on a woman's hormone panel, why each test is there, when it should be drawn, and what the results can and can't tell you.
Timing Isn't a Detail, It's the Whole Ballgame
Before we talk about which hormones to measure, we have to talk about when.
If you're still cycling, your estradiol and progesterone are moving targets. Estradiol climbs through the follicular phase, spikes right before ovulation, dips, then rises again. Progesterone sits near zero for the first half of your cycle and only becomes meaningful after ovulation. Draw progesterone on cycle day 5 and it will look low. Of course it does. It's supposed to be low. That doesn't mean anything is wrong, and it doesn't mean anything is right either. It's simply an uninformative number.
For a cycling woman, I generally want the panel drawn on day 19 to 22 of a 28-day cycle, roughly a week after ovulation, which is when progesterone peaks. If cycles run longer or shorter, we adjust. If cycles have gotten unpredictable, which is the whole story in perimenopause, we sometimes draw twice a few weeks apart or lean harder on symptoms than on numbers.
Perimenopause deserves its own warning label here. The 2011 Stages of Reproductive Aging Workshop consensus, summarized by Siobán Harlow and colleagues in a 2012 paper in the Journal of Clinical Endocrinology and Metabolism, laid out the staging system most of us use for the menopause transition. One of its clearest messages is that hormone levels during the transition are erratic enough that a single draw shouldn't be used to stage anyone. Bleeding patterns, the group concluded, are the most reliable marker. The blood work supports the picture. It doesn't replace it.
That same group's work drew on data from the Study of Women's Health Across the Nation, including a 2011 analysis by John Randolph and colleagues tracking how FSH and estradiol actually change across the transition. What those curves show is that the steep part of the change happens in a fairly tight window around the final menstrual period, and that before you get there, the numbers wobble considerably. A woman in early perimenopause can produce more estrogen in a given month than she did at 32. The next month she can crash. Testing her once and declaring her hormonally fine is like checking the weather on one afternoon and describing the climate.
The Core Sex Hormone Panel
Here's what I want to see, and why.
- Estradiol (E2). The dominant estrogen in your reproductive years. Ask for a sensitive assay, especially if you're postmenopausal, because standard assays get imprecise at low levels and that's exactly the range where precision matters.
- Progesterone. Drawn in the luteal phase or it tells you nothing. Progesterone matters for sleep, for mood, and for balancing estrogen's effect on the uterine lining.
- FSH and LH. These are the pituitary's signals to the ovary. As ovarian responsiveness falls, the pituitary shouts louder, so FSH rises. A persistently elevated FSH in a woman over 45 with changing cycles supports the clinical picture, though a single normal FSH absolutely does not rule out perimenopause.
- Total and free testosterone. Yes, women make testosterone, and yes it declines. It matters for libido, muscle, motivation, and bone. Free testosterone is the fraction actually available to tissue, which is why total alone can mislead.
- SHBG. Sex hormone binding globulin is the protein that grabs onto testosterone and estradiol and keeps them out of circulation. Oral estrogen raises it. Thyroid disease moves it. Insulin resistance lowers it. If SHBG is high, you can have a respectable total testosterone and still have very little free hormone doing anything useful.
- DHEA-S. An adrenal precursor hormone that declines steadily with age and gives me a rough read on adrenal output.
That's the core. Six or seven tubes, and already far more information than the estradiol-and-TSH pairing that most women get handed.
The Thyroid Panel That Isn't Just a TSH
If I could change one habit in primary care, it might be this one. TSH alone is a screening test, not a thyroid evaluation.
TSH is a pituitary hormone. It tells you what your pituitary thinks about your thyroid status. Useful, certainly. But it doesn't tell you how much thyroid hormone you're actually making, whether you're converting the storage form to the active form, or whether your immune system is quietly attacking the gland.
What I order: TSH, free T4, free T3, reverse T3, and thyroid antibodies (TPO and thyroglobulin). The antibodies matter enormously, because Hashimoto's thyroiditis can smolder for years with a TSH that stays inside the reference range while symptoms build. Finding positive antibodies changes how closely I watch someone, even when I'm not treating yet.
There's a real debate about the upper limit of normal for TSH. Most labs report anything under about 4.5 as normal. Plenty of endocrinologists argue the practical ceiling should sit closer to 2.5. I'm not going to pretend that argument is settled, and I'm certainly not going to treat a number in the absence of symptoms. But if you're fatigued, cold, constipated, losing the outer third of your eyebrows, and your TSH is 4.2 with positive antibodies, "normal" is doing a lot of work in that sentence.
The thyroid connection also matters because thyroid symptoms and perimenopause symptoms overlap almost completely. Fatigue, weight change, brain fog, hair thinning, low mood, poor temperature regulation. Both conditions become more common in a woman's forties. Chasing one while ignoring the other is how people end up on three years of unhelpful treatment.
The Labs Nobody Orders and Everybody Needs
Hormones don't operate in a vacuum. A few unglamorous tests round out the picture.
Fasting insulin and hemoglobin A1c. Insulin resistance changes how your body handles sex hormones, and it accelerates during the menopause transition. Fasting glucose alone will look fine for years while insulin climbs to keep it there. Ordering insulin catches the problem while it's still easy to reverse.
hs-CRP. A general marker of inflammatory tone. Not diagnostic of anything by itself, but a useful gauge of the background environment your tissues are sitting in.
Vitamin D, ferritin, and B12. Low ferritin from years of heavy periods is one of the most common causes of fatigue I see in perimenopausal women, and it's routinely missed because hemoglobin stays normal long after iron stores are depleted. A normal CBC does not mean your iron is fine.
A full lipid panel with particle sizing if available. Cardiovascular risk shifts meaningfully after menopause. Getting a baseline while you're still in the transition is worth doing.
A complete metabolic panel and CBC. Basic, boring, necessary. Liver and kidney function affect hormone metabolism and affect what I can safely prescribe.
How to Read Your Own Results Without Losing Your Mind
Two things trip women up when they get their numbers back, and they pull in opposite directions.
The first is the reference range problem. Lab reference ranges are built from the population that gets tested at that lab, which is not a population of thriving 46-year-olds. Being inside the range means you're statistically unremarkable. It doesn't mean you feel good, and it isn't a treatment goal.
The second trap is the opposite error, and I see it just as often. Some clinics chase numbers into ranges that have no outcome data behind them and treat a lab value as if it were a symptom. That's not better medicine, it's just a different way of ignoring the patient. What I care about is whether you sleep, whether you can think clearly at 3 p.m., whether you have the energy for the things you actually want to do. Labs help me get you there. They aren't the destination.
So when we sit down with your results, I'm looking at patterns rather than individual values. High SHBG with a mid-range total testosterone and low libido tells a coherent story. A low progesterone with fragmented sleep and worsening PMS tells another. FSH of 40 alongside cycles that have gone from 28 days to 45 tells a third. None of those conclusions come from one number.

What to Ask For
You don't need a functional medicine practice to get most of these tests. You need a clinician willing to order them and a reason to draw them at the right time. If you're going to your regular doctor, ask specifically for estradiol, progesterone drawn in the luteal phase, FSH, total and free testosterone, SHBG, DHEA-S, a full thyroid panel including free T3 and antibodies, fasting insulin, ferritin, and vitamin D. Write it down. Bring the list.
And know what you're signing up for on the back end. A panel like this generates a lot of data, and data without interpretation just becomes a thicker folder. The value isn't in the draw. It's in someone sitting with you for forty minutes, matching what the numbers say against what your body has been telling you, and building a plan from the overlap.

When in my cycle should I get my hormone levels drawn?
If you're still cycling, timing changes the answer completely. How often do I need blood work?

Should I get my thyroid checked after having a baby?

Postpartum thyroiditis affects about 7 to 8 percent of women, often shows up between 1 and 8 months after delivery, and looks like every other postpartum complaint (fatigue, mood changes, weight issues, hair loss). A full panel with free T3, free T4, TSH, and TPO antibodies takes the mystery out of it. Don't accept a single TSH reading as the answer if you still feel terrible.</p>
My joints started aching in my late 40s but my arthritis labs are all normal. What's going on?
This is one of the most common stories I hear, and a negative rheumatology workup doesn't mean nothing's wrong. Estrogen acts as a brake on inflammation inside your joints, and when estradiol starts swinging and falling in perimenopause, that brake comes off. The inflammation is happening locally in the joint tissue, so it often doesn't show up on CRP, ESR, or rheumatoid factor at all.
