A patient told me last month that she'd made it through an entire school year without catching whatever her kids kept dragging home. First time in a decade. She'd lost about forty pounds on a GLP-1 medication over that same stretch, and she wanted to know if the two things were connected or if she'd just gotten lucky.
I couldn't tell her they were connected. One person, one year, is an anecdote and I try not to build medicine out of anecdotes. But her question is a lot more interesting than she realized, because there's a genuine biological story here that most people never hear. GLP-1 is a gut hormone. It always was. And your intestine happens to be where most of your immune system lives.
So the gut, immunity, and GLP-1 aren't three separate topics that happen to share a blog post. They're three views of the same piece of anatomy. Let me walk you through it, and then get to the practical part, which is the thing I actually spend my time correcting in clinic.
Your Gut Is Where Most of Your Immune System Lives
Here's a number that surprises people: something on the order of seventy percent of your immune tissue sits in and around your intestinal wall. There's a name for it, gut-associated lymphoid tissue, and it makes sense once you think about what your gut actually is. It's a tube running through the middle of you that is technically open to the outside world at both ends. Everything you swallow, every microbe that rides in on it, meets your body for the first time across a single layer of intestinal cells.
That layer is doing something remarkably difficult. It has to let nutrients through while keeping bacteria, toxins, and undigested proteins out. The cells are stitched together by tight junctions, and they're coated in a mucus layer that acts as a first line of defense. Behind them sits an enormous concentration of immune cells whose job is to sample what's coming through, tolerate what's harmless, and mount a response against what isn't. Getting that discrimination right is most of what your immune system does all day.
Your microbiome is the third player. The bacteria in your colon aren't passengers. They ferment the fiber you eat and produce short-chain fatty acids, particularly butyrate, which is the preferred fuel for the cells lining your colon. Butyrate feeds the barrier, supports regulatory immune cells, and helps keep the whole system calm rather than reactive. When fiber intake drops, that fermentation slows, butyrate production falls, and the barrier gets a little leakier. Bacterial fragments like lipopolysaccharide cross over into circulation in small amounts, and the immune system responds to that the way it responds to any bacterial signal: with inflammation.
That's the mechanism behind what researchers call metabolic endotoxemia, and it's one of the more compelling explanations we have for why obesity and chronic low-grade inflammation travel together so reliably. It's not that fat tissue is inherently inflammatory in isolation. It's that a lot of what drives the inflammation starts at the gut wall.
GLP-1 Was a Gut Hormone Long Before It Was a Prescription
Now here's where the medication comes in, and where I think the public conversation misses the point entirely.
Glucagon-like peptide-1 is made by L-cells, which are specialized enteroendocrine cells concentrated in your distal small intestine and colon. When food arrives, these cells release GLP-1, which then slows gastric emptying, stimulates glucose-dependent insulin release, suppresses glucagon, and signals satiety to the brain. That's the incretin effect, and it's the reason a glucose load taken by mouth produces a bigger insulin response than the same glucose given intravenously. Your gut is telling your pancreas what's coming.
What fewer people know is what else triggers L-cells. Short-chain fatty acids, the ones your bacteria make from fiber, are among the signals that stimulate GLP-1 secretion. So the fiber you eat feeds the bacteria, the bacteria make butyrate and propionate, and those metabolites prompt your own intestine to release the very hormone we're now prescribing as a weekly injection. A high-fiber diet is, in a modest and entirely natural way, a GLP-1 strategy.
Semaglutide and tirzepatide are engineered to survive in the bloodstream far longer than native GLP-1, which breaks down within minutes. They're producing a pharmacologic version of a physiologic signal your gut already knows how to send. I think that framing matters, because patients often carry a vague sense that they're taking something foreign and unnatural. They're not. They're amplifying a system they already have.
What Happens to Inflammation on GLP-1 Therapy
If the gut-immune connection is real, we'd expect these medications to move inflammatory markers. They do, and the data on this is better than most people assume.
The clearest look comes from an exploratory analysis by Verma and colleagues published in EClinicalMedicine in 2022, which pooled data from the STEP 1, 2, and 3 trials. These were 68-week randomized, double-blind, placebo-controlled studies in adults with overweight or obesity, with and without type 2 diabetes. Participants on semaglutide 2.4 mg saw C-reactive protein fall by 39 to 48 percent relative to placebo across the three trials. CRP is the workhorse inflammation marker we use in clinic, and it's a recognized predictor of cardiovascular risk. Enough participants dropped into a lower CRP-defined risk category that the shift was measurable at the population level. The reductions tracked alongside weight loss and correlated with improvements in waist circumference, fasting insulin, and insulin resistance.
A 2024 systematic review and meta-analysis by Masson and colleagues in Frontiers in Cardiovascular Medicine widened the lens considerably. Thirteen randomized trials, more than twenty-six thousand patients, and the finding held: semaglutide was associated with lower CRP compared with both placebo and active comparators. The effect showed up whether the drug was injected or taken orally, and whether or not patients had diabetes. Their conclusion was that reduced inflammation is likely one of the mechanisms behind the cardiovascular benefit seen with this class.
I want to be careful about what that does and doesn't prove. Lower CRP is not the same as a stronger immune system, and none of this tells us my patient stopped catching her kids' colds because of her prescription. Weight loss by itself lowers CRP, so untangling drug effect from weight effect is genuinely difficult. What we can say is that these medications are associated with a meaningful reduction in systemic inflammation, that the effect is consistent across large populations, and that it's biologically plausible given where GLP-1 comes from in the first place.
The Fiber Problem Nobody Warns You About
Now the part I actually correct in clinic almost every week.
When appetite drops sharply, fiber is usually the first casualty. Vegetables, legumes, whole grains, and fruit are bulky. They take up stomach volume that patients on these medications suddenly don't have. Meanwhile the same patients are (correctly) prioritizing protein to protect muscle. So the plate shrinks, protein holds its place, and fiber quietly disappears. Add slowed gastrointestinal transit from the medication itself, and constipation becomes one of the most common complaints I hear.
A 2026 review by Yoon and colleagues in the International Journal of Molecular Sciences laid this out directly. The authors describe how appetite suppression, reduced meal volume, and delayed transit on GLP-1 receptor agonist therapy can cut dietary fiber intake and, with it, the fermentable substrate reaching the colon. Less substrate means less short-chain fatty acid production, which means less butyrate for the cells maintaining your intestinal barrier. Their argument is that viscous soluble fibers, psyllium in particular with glucomannan as a secondary option, can help close that gap and support bowel function and barrier integrity during weight-loss therapy. They're appropriately measured about it, framing some of the downstream claims as hypothesis-generating rather than settled.
But the core problem they identify is not hypothetical at all. I see it constantly. A patient is losing weight beautifully, inflammatory markers are heading the right direction, and their fiber intake has fallen to maybe eight grams a day when it should be north of twenty-five. That's a microbiome getting starved in the middle of an otherwise successful treatment. It's the one place where the medication working well can quietly create a new problem.
What We Do About It
None of this is complicated to fix, but it does require someone paying attention to it.
I set a fiber target the same way I set a protein target. For most patients that's 25 to 35 grams a day, and if food volume makes that unrealistic, we supplement, usually with psyllium, started low and increased slowly with plenty of water. Ramping fiber too fast in someone whose gut is already moving slowly is a good way to make them miserable, so we take our time.
We front-load fiber into the meals people can still finish. Berries, chia, ground flax, beans, and cruciferous vegetables give you the most fermentable fiber per bite, which matters a great deal when total bites are limited. I also encourage fermented foods, plain yogurt, kefir, kimchi, sauerkraut, not because I think they'll rebuild a microbiome single-handedly but because they're cheap, low-risk, and add microbial diversity to a diet that's shrinking.
Hydration gets its own conversation, because fiber without water makes constipation worse rather than better. And we track bowel habits at every visit. It's not a glamorous question, but a patient who hasn't had a normal bowel movement in five days is telling me something about their gut ecology, not just their comfort.
When somebody's inflammatory markers aren't improving the way I'd expect, or their gut symptoms are out of proportion to their dose, that's when we look harder. Sometimes it's thyroid. Sometimes it's a food intolerance that was always there and got unmasked. Sometimes we're just moving too fast on titration. Begin with about five grams once a day in a full glass of water and work up slowly, because going straight to a big dose is how people end up bloated and convinced fiber doesn't work. I'd be cautious with inulin and chicory-root prebiotic powders since they ferment aggressively and can make GLP-1 bloating worse.
When is constipation on a GLP-1 serious enough to call the doctor?
Call us if you've gone more than five to seven days without a bowel movement, especially with a distended belly, worsening pain, or vomiting.
