A patient sat down in my office last spring and told me something I've now heard some version of a few hundred times. He was 54, worked in commercial real estate, used to play tennis four days a week, and had spent the past two years slowly quitting everything he liked doing. His knees ached. His shoulders ached. Getting out of the car after a long drive took a beat longer than it used to. His orthopedist had shown him X-rays with mild joint space narrowing, told him it was garden-variety osteoarthritis, and offered him a cortisone shot and an anti-inflammatory. Neither did much. What brought him to me wasn't his joints at all. It was fatigue. But when his labs came back with a total testosterone of 240 ng/dL, he asked the question that a surprising number of men eventually ask: if we fix the testosterone, will my joints stop hurting?

It's a fair question, and the honest answer is more interesting than a simple yes or no. I'm Dr. Farhan Abdullah, an internal medicine physician and the medical director at Magnolia Functional Wellness in Southlake. I still work as a hospitalist, which means I spend a good chunk of my week around people whose mobility has already failed them. That perspective shapes how I think about joint pain in men who are still in their forties, fifties, and sixties. There's a window where the trajectory can still be bent. Testosterone is one lever in that window. It just isn't the lever most men assume it is.

What's Actually Going Wrong Inside an Arthritic Joint

Most people picture osteoarthritis as pure mechanical wear. Cartilage grinds down, bone rubs bone, pain follows. That model isn't wrong, but it's badly incomplete, and the incompleteness is exactly where hormones enter the picture.

Modern rheumatology understands osteoarthritis as a whole-joint disease with a real inflammatory component. Yes, cartilage degrades. But alongside that, the synovial lining gets irritated and thickened, fluid accumulates (what radiologists call effusion-synovitis), and the bone underneath the cartilage develops lesions visible on MRI. Inflammatory signaling molecules circulate through all of it. Tumor necrosis factor alpha, interleukin-1 beta, and interleukin-6 are the usual suspects. These cytokines don't just sit there. They actively drive the enzymes that break cartilage down, and they sensitize the nerve endings around the joint so that a given amount of mechanical stress registers as more pain.

That last part deserves emphasis, because it explains something clinicians see constantly. Two men can have nearly identical X-rays and radically different levels of suffering. The difference often isn't in the cartilage. It's in how inflamed and how sensitized the surrounding tissue is. Which means anything that shifts your systemic inflammatory tone has a plausible path to changing how your joints feel, even if it never touches the cartilage itself.

So what does testosterone have to do with inflammatory tone? Quite a bit, as it turns out.

Testosterone Is an Immune-Modulating Hormone

We tend to file testosterone under muscle, libido, and mood. Those are real, but they undersell it. Testosterone receptors show up on immune cells, on synovial tissue, on chondrocytes (the cells that maintain cartilage), and on bone. It's an anabolic hormone in the broad sense, meaning it favors building over breaking down, and that bias extends past skeletal muscle into connective tissue.

The most direct human evidence I point patients toward is a randomized, placebo-controlled crossover trial by Malkin and colleagues, published in the Journal of Clinical Endocrinology and Metabolism in 2004. They took 27 men with symptomatic androgen deficiency and measured what happened to their inflammatory cytokines on testosterone replacement versus placebo. The results were fairly clean: TNF-alpha dropped significantly, IL-1 beta trended down, and the anti-inflammatory cytokine IL-10 went up. Total cholesterol fell as a bonus. The authors concluded that testosterone replacement shifts the cytokine balance toward a less inflammatory state.

Now, that trial was designed around cardiovascular risk, not knees. Nobody measured WOMAC scores or ran MRIs. But the mechanism is the mechanism. If restoring testosterone in a deficient man lowers the exact cytokines that drive cartilage breakdown and pain sensitization, that's not nothing. It's a biologically coherent reason to expect some joint benefit, and it lines up with what I hear from patients months into therapy: less morning stiffness, faster recovery after they push themselves, joints that feel less hot and irritable.

Coherent mechanism isn't proof, though. Medicine is full of elegant mechanisms that evaporated when somebody ran the outcome trial. So what do we see when we actually look at joint pain and testosterone levels in real populations?

What the Human Joint Data Shows (and Where It Gets Complicated)

Two studies frame this well, and they don't agree perfectly, which is exactly why I like using both.

The first is a 2025 analysis published in the Scandinavian Journal of Pain by Masoud and colleagues, drawing on 1,764 participants from the Qatar Biobank. They ran eleven clinical lab markers against WOMAC scores, which is the standard validated questionnaire for osteoarthritis pain, stiffness, and physical function. Testosterone came out significant across the board. Higher testosterone levels tracked with less pain, less stiffness, and less physical dysfunction, and the associations held up statistically after adjustment. Fibrinogen, an inflammatory marker, moved in the opposite direction. The authors' conclusion was that testosterone may be protective against pain, stiffness, and physical dysfunction.

The second is older and more granular. Jin and colleagues, writing in Osteoarthritis and Cartilage in 2017, followed 200 people with symptomatic knee osteoarthritis for two years with serial MRI and serial sex hormone panels. In women, lower testosterone (along with lower estradiol and progesterone) was associated with more effusion-synovitis, and testosterone was inversely associated with knee pain. In men, they found no consistent associations.

Read those two together and you get an honest picture. There's a real signal connecting testosterone to joint symptoms. It's cross-sectional and observational rather than interventional, meaning we can't say testosterone caused the difference. And the male-specific structural data is genuinely mixed. In a cohort where most men still had testosterone somewhere in the reference range, moving around within that range didn't obviously change their MRIs.

Here's how I reconcile that clinically. The men I treat aren't the men in those cohorts. A guy at 240 ng/dL with symptoms isn't operating inside the physiologic range at all. Studying variation within normal tells you very little about what happens when you correct genuine deficiency. That's a different question, and to my knowledge nobody has yet run the randomized trial that answers it directly for joint outcomes.

The Indirect Effects Might Matter More Than the Direct Ones

Let me make an argument that gets overlooked in the mechanism debate. Even if testosterone's direct anti-inflammatory effect on the joint capsule turned out to be modest, restoring it in a deficient man can change joint pain substantially through paths that have nothing to do with cytokines.

Consider what low testosterone does to a body. Muscle mass declines, particularly the fast-twitch fibers around the hip and knee that decelerate you when you walk downhill or step off a curb. Visceral fat accumulates, and visceral fat is metabolically active tissue that pumps out inflammatory signals on its own. Every extra pound on the frame multiplies through the knee at roughly three to four times body weight during normal gait. Sleep quality degrades, and poor sleep measurably lowers pain thresholds. Mood and motivation flatten, so activity drops, so the muscles supporting the joint weaken further.

That's a self-reinforcing loop, and it produces a very specific kind of patient: someone whose imaging looks like moderate arthritis but whose function looks like severe arthritis. Correct the hormone and you're not treating cartilage. You're rebuilding the muscular scaffolding that unloads the joint, dropping the inflammatory fat mass, restoring the sleep that regulates pain perception, and giving the person back enough drive to actually do their physical therapy. Those things compound.

One of my patients described it better than I can. Six months in, he told me he hadn't noticed his knees getting better so much as he'd noticed he stopped thinking about them. He was back to walking the trails at Bob Jones Park on weekends. Same cartilage. Different life.

What Testosterone Won't Do for Your Joints

This is the part where I'd rather be the doctor who told you the truth than the clinic that sold you a story.

Testosterone does not regrow cartilage. There's no credible human evidence that testosterone replacement therapy reverses established structural osteoarthritis. If you have bone-on-bone changes and a mechanical block to motion, hormone optimization isn't going to restore that joint surface. Be skeptical of anyone who implies otherwise.

It's also not a treatment for rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, or any other autoimmune inflammatory arthritis. Those conditions require proper rheumatologic management with disease-modifying drugs, and delaying that care while you experiment with hormones can cost you joint architecture you don't get back. Men with inflammatory arthritis frequently do run low testosterone, partly from chronic inflammation and partly from steroid exposure, and correcting it can help how they feel. It's adjunctive. It's not the treatment.

Testosterone won't fix a torn meniscus, a full-thickness rotator cuff tear, or a labral injury. Structural damage needs structural evaluation.

And it's not risk-free. TRT raises hematocrit, which needs monitoring. It suppresses fertility, which matters for younger men. It requires ongoing PSA surveillance and blood pressure attention. Anyone prescribing it as a casual joint-pain remedy without a proper workup isn't practicing good medicine.

How I Actually Approach This at Magnolia

When a man in his fifties comes to me with aching joints and a suspicion that his hormones are involved, the sequence is fairly consistent.

First, we establish whether he's actually deficient. That means total and free testosterone drawn in the morning on two separate occasions, plus SHBG, LH, FSH, estradiol, a metabolic panel, hemoglobin A1c, high-sensitivity CRP, vitamin D, and thyroid function. A single borderline testosterone doesn't diagnose anything. I also want the inflammatory picture, because if his hs-CRP is elevated I need to know whether that's metabolic, autoimmune, or something else entirely before I start attributing symptoms.

Second, we characterize the joints properly. Which joints, what pattern, morning stiffness lasting how long, any swelling, any imaging already done. Symmetric small-joint involvement with an hour of morning stiffness sends him to rheumatology, not to my hormone protocol. Asymmetric large-joint pain in a former athlete with a metabolic profile heading the wrong direction is a different animal.

Third, if he's genuinely hypogonadal, we treat that on its own merits and set honest expectations about the joints. I tell men that energy and mood typically shift in the first four to six weeks, body composition takes three to six months, and any joint benefit is usually a downstream effect of those changes rather than something they'll feel in week two.

Fourth, and this is where I think functional and regenerative medicine actually earns its keep, we treat the joint directly rather than hoping the hormone does it. Optimized testosterone plus a sedentary lifestyle and an untreated knee is a wasted opportunity. Depending on the joint and the pathology, that might mean PRP injections, shockwave therapy, targeted strength work, weight optimization, or some combination. Hormones set the terrain. Local therapy does the local work. Running both together produces results that neither one does alone, and I've watched that play out often enough to be confident in it.

So, does testosterone help arthritis? If you're genuinely deficient, restoring it will likely make your joints feel better, mostly by lowering systemic inflammation, rebuilding the muscle that protects the joint, and giving you back the capacity to move. If your testosterone is already normal, chasing it won't solve your knee. And in either case, it belongs inside a real plan rather than standing in for one.
