Blood Pressure Monitoring: Essential for TRT Patients

A patient of mine, mid-forties, three months into testosterone therapy, walked into his follow-up practically glowing. Energy back. Sleeping through the night. Down two pant sizes. Then the medical assistant put the cuff on him and we got 152 over 96. He looked at the number and shrugged. "I feel great, though." That sentence right there is the whole problem with blood pressure. It doesn't announce itself. You don't feel 152 over 96. You feel it fifteen years later, in a cardiology office, being told about your left ventricle.

I'm an internal medicine physician. Before I opened Magnolia Functional Wellness here in Southlake, and while I still work shifts at a hospital in Dallas, I spent a lot of time admitting people whose hypertension had quietly been doing damage for a decade. So when men come to me for testosterone replacement therapy, blood pressure isn't an afterthought on the intake form. It's a vital sign we track as seriously as we track total testosterone, hematocrit, and PSA. Not because TRT is dangerous. Because good TRT is monitored TRT, and blood pressure is one of the few things that can go wrong silently.

Let's talk about what the research actually shows, what I watch for in my own patients, and what you should be doing at home between visits.

Does Testosterone Actually Raise Blood Pressure?

Here's where the honest answer is more interesting than a yes or no. The signal is real, it's small on average, and it's not evenly distributed across patients.

Two industry-sponsored trials looked at this question directly using 24-hour ambulatory blood pressure monitoring, which is a far better measurement than a single reading in a clinic hallway. The first, published in Endocrine Practice in 2024 by Efros and colleagues, followed 168 hypogonadal men on a transdermal testosterone system for 16 weeks. The mean change in 24-hour systolic pressure was 3.5 mmHg. The second, a 2025 study in Andrology by Weber and colleagues, enrolled 246 men on testosterone gel 1.62% and found a mean 24-hour systolic increase of 1.9 mmHg over the same 16 weeks. Neither trial could formally rule out a drug effect, since the confidence intervals edged past their pre-specified 3 mmHg noninferiority margin.

So, a couple of millimeters of mercury. Is that a crisis? For most men, no. A 2 mmHg average shift is smaller than what you'd get from a stressful commute up 114 or a poor night's sleep. But averages hide people. In both of those trials, the changes were larger during daytime hours than at night, and larger in the subgroups who already had hypertension or diabetes. The Weber study found noninferiority held in men without hypertension or diabetes and did not hold in men who had them. That's the pattern I care about clinically. Testosterone doesn't hit everyone the same way, and the men most likely to see a meaningful bump are the ones who arrived with the least cardiovascular margin to spare.

And then there's the reassurance from the biggest trial we have. The TRAVERSE trial, published in the New England Journal of Medicine in July 2023 by Lincoff and colleagues, randomized more than 5,000 middle-aged and older men with hypogonadism and either established cardiovascular disease or high cardiovascular risk. Testosterone gel was noninferior to placebo for major adverse cardiac events. That's a genuinely important finding, because for a solid decade the field operated under a cloud of uncertainty about whether TRT caused heart attacks. TRAVERSE says, in the population studied, it didn't.

What I tell my patients is this: the cardiovascular event data is reassuring, the blood pressure data says "pay attention," and those two statements are not in conflict. A small average pressure increase over decades still matters for stroke and kidney risk, even if a two-year trial doesn't catch it.

The Hematocrit Connection Nobody Mentions

Now for the part I find most clinically useful, and the part most TRT clinics gloss over entirely.

Testosterone stimulates erythropoiesis. Your bone marrow makes more red blood cells. Hematocrit climbs. This is expected, and by itself it isn't automatically a problem. But thicker blood means higher viscosity, and higher viscosity means the heart works against more resistance. That's not hand-waving physiology. There's data connecting the two.

A double-blind, randomized, placebo-controlled trial by Olesen and colleagues in the Journal of Hypertension studied opioid-treated men with relative hypogonadism, randomizing them to testosterone or placebo for six months. Systolic office pressure rose 6.2 mmHg in the testosterone group and fell 7.0 mmHg in the placebo group, a between-group difference of 13.2 mmHg. That's not trivial. What makes the study valuable, though, is the mechanism it uncovered: in the testosterone group, every 10 mmHg rise in systolic pressure tracked with a hematocrit increase of 0.3 percentage points. No such relationship existed in the placebo arm. The men most affected were those with higher BMI and red cell levels already sitting in the upper normal range.

This is a specialized population and I don't want to over-extrapolate from opioid-treated men to the average guy on TRT. But it lines up with what I see in practice. When a patient's blood pressure starts creeping up on therapy, the first lab I look at isn't his testosterone level. It's his hematocrit. If it's marched from 44 to 53 percent over six months, I've usually found my culprit, and the fix is often mechanical rather than pharmacologic: lower the dose, switch to smaller more frequent injections instead of a large weekly bolus, address sleep apnea if it's lurking, and if needed, therapeutic phlebotomy or a straightforward blood donation.

Men on TRT are often surprised to hear that giving blood is part of their protocol. It's one of the more elegant interventions in medicine. You solve a viscosity problem, you help the local blood supply, and you're out in forty-five minutes.

What Proper Monitoring Actually Looks Like

Here's the protocol I use at Magnolia Functional Wellness, and honestly, it's not complicated. It just requires somebody to actually do it.

Before we start. Nobody begins testosterone replacement therapy in my practice without a documented baseline blood pressure, ideally an average of readings taken on more than one occasion. I want baseline hematocrit and hemoglobin, a lipid panel, a metabolic panel with kidney function, and PSA in age-appropriate men. If somebody's sitting at 150 over 95 untreated on day one, we manage that first. Starting TRT on top of uncontrolled hypertension is putting a new roof on a house with a cracked foundation.

Home monitoring. This is non-negotiable and it's the highest-yield thing a patient can do. Get a validated upper-arm cuff. Not a wrist device, not a smartwatch estimate. Check twice a week in the morning before caffeine, sitting for five quiet minutes first, feet flat, back supported, arm at heart level, cuff on bare skin. Take two readings a minute apart and record both. That log tells me more in ninety seconds than an entire clinic visit does, because it captures your real life rather than the moment your pressure spiked because you hit traffic on the way over.

Follow-up labs. At six to eight weeks, again at three months, then every six months once things are stable. Hematocrit is the number I'm watching for drift. I generally get uncomfortable above 52 to 54 percent, and I'd rather intervene at the trend than at the threshold.

When to escalate. If home readings consistently sit above 130 over 80, we're having a conversation. If they're above 140 over 90, we're changing something. That might mean dose reduction, a delivery-method change, adding an antihypertensive, or digging into the things underneath: sleep apnea, alcohol intake, sodium load, chronic NSAID use, or the stress of a job you haven't taken a real weekend from in two years.

A note on white coat readings

Roughly one in five people has meaningfully higher pressure in a medical office than they do at home. If your clinic readings are always high and your home log is always fine, you may not need treatment at all. And the reverse exists too. Masked hypertension, where office readings look great and home readings don't, is arguably more dangerous because it goes undetected. This is exactly why I ask for the home log rather than trusting a single number from my own exam room.

The Things That Move the Needle More Than the Testosterone Does

Something worth sitting with: in most of my patients whose blood pressure rises on TRT, the testosterone is a contributor rather than the cause. The bigger drivers are usually hiding in plain sight.

Sleep apnea is the one I catch most often. Testosterone can worsen it, untreated apnea drives hypertension hard, and a startling number of men have it without knowing. If you're snoring, waking unrefreshed, or your partner has mentioned you stop breathing, get a home sleep study before you blame the testosterone.

Then there's the gym factor. Men who feel good on TRT tend to train harder, which is the point. But heavy resistance training plus pre-workout stimulants plus creatine plus a few extra pounds of retained water can nudge readings up in a way that has nothing to do with the hormone itself. Same with the estrogen conversion story: some men aromatize enough that fluid retention becomes noticeable, and managing that thoughtfully (not by crushing estradiol to zero, which is its own mistake) often settles things down.

Alcohol deserves a mention too. Two drinks a night is not neutral for blood pressure, and I've had more than one patient discover that his numbers normalized when he cut back, not when we changed his protocol.

Weight is the other lever. If a man is carrying an extra thirty pounds of visceral fat, losing even ten percent of body weight will do more for his blood pressure than almost anything I can prescribe. That's part of why hormone optimization works best when it's part of an actual plan rather than a standalone injection.

Why This Matters More Than the Number Suggests

Testosterone therapy done well is one of the most satisfying things I do. Men get their focus back, their drive back, their willingness to engage with their own lives back. I've watched patients go from dreading the day to coaching their kid's team again. That's real, and I'm not interested in scaring anyone away from it.

What I am interested in is the difference between TRT that's monitored and TRT that's just dispensed. The clinics that hand out vials without checking a hematocrit or a blood pressure aren't practicing medicine, they're running a subscription service. The whole value of physician-supervised therapy is that somebody's watching the parameters that don't produce symptoms until they've already produced damage.

Blood pressure is the quietest of those parameters. It costs about forty dollars for a decent home cuff and roughly two minutes twice a week to track it. Against that, you're protecting your kidneys, your brain, your heart, and the eyesight you'd like to still have at eighty. That's an absurdly good return on effort.

If you're on testosterone therapy and nobody has asked you about your blood pressure in the last six months, that's a gap worth closing. And if you're considering starting, build the monitoring into your expectations from day one rather than bolting it on later. Erythrocytosis (elevated hematocrit): testosterone's erythropoietic effect can raise red blood cell concentration. Hematocrit above 52–54% increases clotting risk meaningfully. This is monitored at every follow-up; dose reduction or therapeutic phlebotomy addresses elevation before it becomes a clinical problem. Estradiol elevation: aromatization of testosterone to estradiol can cause water retention, mood changes, and erectile dysfunction if unmanaged. Estradiol is checked at follow-up visits; anastrozole is prescribed when elevation is confirmed and symptomatic. Testicular atrophy and sperm suppression: addressed with hCG when fertility preservation matters. Skin reactions at injection or application sites: typically mild and transient.
